Thank you for choosing Texas Orthopedics for your orthopedic care. Once you have made your initial appointment, we encourage you to download and complete the patient history and intake form prior to your first visit and bring it with you. If the patient is a minor that needs to be seen without a parent or guardian, the “Authorization to Treat a Minor” form will need to be completed in order for us to treat a minor.

Medical Records Release

If you require a copy of your medical record or require the completion of a Disability/FMLA Form, please complete the associated form (links to forms provided below). Please be advised that requests for Medical Records and requests for Disability Form Completion require separate Authorization Forms to be completed. Further information relating to process and fees for requests can be found on the second page of the forms. For any additional questions, please contact or Release of Information department at 512-439-1000, Option ‘2’, then Option ‘1’. If you are requiring expedited copies of records for Continuation of Care purposes, your medical care provider may request records directly from our offices via phone at 512-439-1000, Option ‘2’, then Option ‘1’.

Dr. Mukai's regular office hours vary by location as follows:

Central Austin:

• Tuesday & Thursday: 1 - 5 pm
• Wednesday: 1 - 5 pm

South Austin:

• Monday: 8 am - 5 pm
• Wednesday & Friday: 8 am - 12 pm

In the event of a life-threatening medical emergency, call medical emergency services at 911 immediately. DO NOT WAIT! Inform the emergency room staff that you are a patient of Dr. Mukai at Texas Orthopedics, and they will contact us.

If you have an urgent medical need and need to talk to Dr. Mukai after our regular hours, please call (512) 323-5465. A doctor is on-call 24 hours a day. Your call will be returned promptly.
